    I arrived at Safe Haven on 02/28/2018 by Referral from Candice Barton LMSW. I spoke with my Case Manager from Presbyterain Night Shelter and I did deliver the Disability Housing Verification Form to the MHMR and the Form is Pending with ADA Coordinator Cathy Stout, (817) 569-4372, and I will verify the documentation as soon as I am Complete of The "ETO" to Safe Haven by True Worth Place, Tarrant County College /Workforce Services and Client/Resident Notification Calendar.
