Documentation Required: Picture ID, Birth Certificates or Social Security Cards for all household members, Verification of address (Lease/Electric Bill, etc.), Referral from an agency Case Manager, State Service Center. The referral shouldandhave the clients name, address, number in household, name of Case Manager,andcurrent date, and name of referring agency on their letterhead.
