Description: CTI is an empirically supported, time-limited case management model designed to prevent homelessness and other adverse outcomes in people with mental illness following discharge from hospitals, prisons and other institutions. CTI is designed to be most effective when individuals are housed in the community and helped with a long term support plan.

We are a collection of main-line Christian church denominations reaching out to the homeless in a long term program to see a homeless person through to independence and a better life.

It was observed that thousands of homeless persons float around "the system" of short term options for the homeless every night, and never escape homelessness. Moreover, it was observed that most homeless persons would love to see themselves in their own house, their own car, and working steadily in a career.

Kingdom Pathways has labored for years in developing a model to help a homeless person achieve the goals listed above. After years of research, and hands on 24/7 experience with the homeless, Kingdom Pathways has developed a long term, community based, comprehensive, professional, and Christian model, using evidence based outcomes approaches, to bring homeless people to independence!

Kingdom Pathways provides a community based approach, with people from the community assisting homeless clients in many competency areas, to help them on their way to independence. Each of the following competency areas are addressed in each homeless client's Individual Program Plan (IPP):

    Chemical Dependency
Mental Health (anxiety, depression, et cetera)
Attention to Medical and Dental Health
Structured Living and Basic Life Skills
    Money Management
    Anger Management
    Relationship Skills
    Sexual abstinence outside of marriage
    Healthy Support Network Development
    Crime Freedom
    Work Ethic
    Community Service
Job Skills (hard and soft skills)
Job Placement Skills (applications, resumes, interviews)
    Cleanliness and Personal Hygiene

Volunteers Requests by Kingdom Pathways Freedom Outreach: YES !!! Lots to do, as we are a comprehensive, long term program for the homeless.

Donation Requests by Kingdom Pathways Freedom Outreach: Meats, vegetables, milk, snacks, breakfest foods, basic needs like toilet paper, cleaning supplies, paper towels, laundry detergent, et cetera. Most anything a homeless shelter would need to provide 3 meals a day in a long term environment to the homeless and hungry.
