Must provides the following information before utilizing the food locker for the first time: Proof of Ledyard/Gales Ferry residency, Name/Date of Birth of all household members, Last four weeks of income (for all household members), Most recent bank statement (for all household members) and Individuals must also sign a Release of Liability Form. .
